Transitional Care Management

When transitioning between different healthcare settings, such as from a hospital to home care or between practitioners, it is crucial to have proper transitional care management to ensure healthcare continuity and promote safe patient transfers. We provide tailored transitional care services to make the transition period stress-free, safe, and with the best possible outcome.

How does Transitional Care Management work?

Transitional care management lasts 30 days, starting from the date of discharge from the hospital or skilled nursing facility and continuing for the next 29 days. We provide interactive contact, non-face-to-face services, and face-to-face visits during this period.

Remote Patient Monitoring (RPM)

When a patient is homebound, getting them to their primary care provider or providing necessary care and tests can be challenging.
Remote Patient Monitoring (RPM) technology allows Mobile HealthCare to monitor patients outside traditional clinical settings. Digital devices enable our providers to collect data on vital signs, symptoms, and other health indicators, which are reviewed in real-time. RPM has been proven to be an effective way to improve patient outcomes, reduce healthcare costs, and increase access to care for those who need it most.

With advancements in healthcare technology, Mobile HealthCare can immediately monitor patients for conditions such as

Mobile HealthCare has partnered with Cincinnati’s post-acute facilities, assisted living communities, and independent living communities to offer early and effective post-acute services.
