Chronic Care Management Services include:
- 9Minimum of 20 minutes of CCM services per month

Transitional Care Management
When transitioning between different healthcare settings, such as from a hospital to home care or between practitioners, it is crucial to have proper transitional care management to ensure healthcare continuity and promote safe patient transfers. We provide tailored transitional care services to make the transition period stress-free, safe, and with the best possible outcome.
How does Transitional Care Management work?
Transitional care management lasts 30 days, starting from the date of discharge from the hospital or skilled nursing facility and continuing for the next 29 days. We provide interactive contact, non-face-to-face services, and face-to-face visits during this period.

Our team assists with the discharge process, developing custom care plans based on discharge instructions and assessments.
