How does Transitional Care Management work?

Transitional care management lasts 30 days, starting from the date of discharge from the hospital or skilled nursing facility and continuing for the next 29 days. We provide interactive contact, non-face-to-face services, and face-to-face visits during this period.

Digital devices enable our providers to collect data on vital signs, symptoms, and other health indicators, which are reviewed in real-time.
